About the role
The Physical Therapist Assistant (PTA) Case Manager Home Health position at AuthoraCare Collective is Full-Time, Monday through Friday, from 8:00am to 5:00pm.
Responsibilities
- Restores function, improves mobility, relieves pain, prevents, or limits permanent physical disabilities of patients suffering from injuries and disease under the direction of the Physical Therapist.
- Adheres to the therapy plan of treatment for a case load of home health patients in collaboration with other members of the interdisciplinary team (IDT).
- Works with the Physical Therapist and Patient Physician to maintain a treatment plan using a variety of methods, including exercises, stretching maneuvers, hands-on therapy, and equipment to ease pain and help increase mobility and endurance.
- Establishes and maintains a therapeutic relationship with patients, including providing community support for resources and education.
- Documents the goals and outcomes of care.
Qualifications
- Bachelor's degree in Physical Therapy or related field.
- Current licensure as a Physical Therapist Assistant in the state of [State] and certification as a Certified Home Health Aide (CHHA) or equivalent.
- Experience in home health care setting preferred.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
