About the role

As the Physical Therapist in Home Health, you will complete assessments and evaluations of patient care needs related to functional status, mobility, endurance, pain, and other medical conditions. Based on this assessment, you will help determine a treatment plan, perform interventions to improve patient well-being, and evaluate progress. This position offers a $5,000 sign-on bonus.

Responsibilities

  • Evaluate the patient's functional status and physical therapy needs.
  • Assist the physician in developing the therapy plan of care.
  • Provide services within the scope of practice as defined by state laws, in accordance with the plan of care and in coordination with the healthcare team.
  • Observe and report the patient's response to treatment and any changes in condition to the supervising nurse and/or physician.
  • Treat patients to relieve pain, develop or restore function, and maintain maximum functioning using acceptable standards of practice.
  • Instruct and inform the healthcare team, patient, and family/caregivers about the plan of care, functional limitations, and progress toward goals.
  • Visit patients according to the Plan of Treatment, complete progress notes for each visit, and submit them per policy.
  • Complete all patient evaluations and develop the PT plan of care within state-specific guidelines.
  • Report outcomes of evaluation, goals, and anticipated projected frequency of care.
  • Function in any home situation regardless of the client's age, race, creed, color, sex, disability, or financial condition.

Requirements

  • Current Physical Therapy licensure in the state of practice.
  • Current driver’s license, vehicle insurance, and access to a dependable vehicle (or public transportation).

Preferred Qualifications

  • Current CPR certification or ability to complete within 90 days of hire.

Pay

Annual total cash compensation for this role assumes full-time employment (40 weekly hours) at full productivity and generally follows the range of $82,048 - $123,072. Total cash compensation includes earnings from per-visit point pay and hourly pay and is based on factors such as local labor markets, education, and work experience. Compensation may increase over time based on productivity and performance.

  • Hourly rate: $39.45 - $59.17
  • Per-visit point rate: $47.34 - $71.00

This role receives two types of compensation depending on the work performed:

  • For visits: Paid per visit point rate, calculated by multiplying the per-visit point rate by the productivity points accrued for various types of visits. Each visit type is assigned productivity points inclusive of direct and indirect patient care activities.
  • For non-visit activities (e.g., orientation): Paid at the hourly rate.

The number of visits performed each week varies based on individual productivity targets and business needs.
