About the Role

We are seeking a licensed Physical Therapist to provide comprehensive services for PACE participants. This role requires a dedicated professional committed to improving seniors' quality of life within a collaborative, person-centered environment.

Responsibilities

• Conduct assessments and develop individualized treatment plans.
• Administer therapy treatments to improve participant mobility and independence.
• Recommend adaptive equipment, home modifications, and safety assessments.
• Educate participants, families, and staff on therapy techniques and safety procedures.
• Collaborate with the interdisciplinary team to develop and update care plans.
• Document evaluations, treatments, and progress accurately and timely.
• Train staff in proper body mechanics and equipment use to ensure safety.
• Participate in team meetings, quality improvement initiatives, and organizational events.
• Maintain professional licensure, certifications, and continuing education requirements.

Requirements

• Advanced degree in Physical Therapy from an accredited institution.
• Current physical therapy license in the state.
• Experience working with elderly or frail populations preferred.
• Ability to perform in-home and community assessments and treatment.
• Excellent communication and teamwork skills.

Skills

• Bilingual in Hmong preferred.
