Responsibilities
- Perform initial evaluation and functional assessment of newly admitted patients and patients returning from outside hospitalization.
- Develop individualized treatment plan based on clinical findings.
- Provide skilled physical therapy interventions for patients.
- Monitor progress and modify interventions as clinically indicated.
- Document in EHR timely and accurately all evaluations, treatment, progress notes, and discharge summaries.
- Educate patients and direct care staff regarding diagnosis, treatment plans, fall and injury prevention strategies.
- Collaborate with physicians and other team members to ensure coordinated care for patients.
- Serve in Safe Patient Handling and Falls committees as needed.
Requirements
- Doctor of Physical Therapy (DPT) from a program registered by New York State Education Department or accredited by CAPTE, or equivalent if educated outside the USA.
- Pass the National Physical Therapy Examination (NPTE) and be licensed to practice Physical Therapy in NYS.
- Minimum one year of experience.
Schedule
20 hours per week, Monday to Friday, 8:00 AM to 4:30 PM (hours are flexible).
Additional Requirements
- State and national criminal background checks are required.
- Vaccinations: Influenza (during season), Hepatitis B, and Covid-19.
- OMH required training (provided by OMH): TRUST and annual standard trainings.
