Responsibilities

  • Perform initial evaluation and functional assessment of newly admitted patients and patients returning from outside hospitalization.
  • Develop individualized treatment plan based on clinical findings.
  • Provide skilled physical therapy interventions for patients.
  • Monitor progress and modify interventions as clinically indicated.
  • Document in EHR timely and accurately all evaluations, treatment, progress notes, and discharge summaries.
  • Educate patients and direct care staff regarding diagnosis, treatment plans, fall and injury prevention strategies.
  • Collaborate with physicians and other team members to ensure coordinated care for patients.
  • Serve in Safe Patient Handling and Falls committees as needed.

Requirements

  • Doctor of Physical Therapy (DPT) from a program registered by New York State Education Department or accredited by CAPTE, or equivalent if educated outside the USA.
  • Pass the National Physical Therapy Examination (NPTE) and be licensed to practice Physical Therapy in NYS.
  • Minimum one year of experience.

Schedule

20 hours per week, Monday to Friday, 8:00 AM to 4:30 PM (hours are flexible).

Additional Requirements

  • State and national criminal background checks are required.
  • Vaccinations: Influenza (during season), Hepatitis B, and Covid-19.
  • OMH required training (provided by OMH): TRUST and annual standard trainings.
