PHASES Community Support Specialist

Summary

Betty Jean Kerr Peoples Health Centers is seeking a Youth Community Support Specialist (CSS) to support individuals at risk of hospitalization or recently discharged from inpatient treatment. The CSS will facilitate activities and interactions to help clients adjust to community life, evaluate client status, develop person-centered plans, and assist clients in achieving their goals.

Requirements

• Bachelor's degree and two years of qualifying experience
• Any four year combination of higher education and qualifying experience
• Four years of qualifying experience, including:
• Delivery of service to individuals with mental illness, substance abuse, or developmental disabilities
• Providing one-on-one or group services with a focus on coping and managing psychiatric, developmental, or substance abuse issues
• Encouraging the use of natural resources and supporting efforts to find and maintain employment
• Absorbing and implementing individualized treatment or person-centered plans
• Valid Class E driver’s license, dependable transportation, and personal vehicle insurance
• Ability to obtain Class E Driver's License
• Maintain personal vehicle insurance

Qualifications

The ideal candidate will have a strong understanding of mental health principles and practices, excellent communication and interpersonal skills, and the ability to work independently and as part of a team. Proficiency in Microsoft Office and familiarity with electronic health records systems is preferred.
