Pharmacy Technician-Medication History, 40hr, Day
This position requires a Massachusetts pharmacy technician registration and national pharmacy certification by the Pharmacy Technician Certification Board (PTCB).
About the role
UMass Memorial Health seeks a highly motivated individual to initiate and expand the services provided in this new role. The role involves performing transition of care (TOC) related tasks such as collecting and verifying allergy and medication history. The TOC liaison will communicate with patients, families, nurses, physicians, and other healthcare professionals to obtain accurate home medication lists upon hospital admission.
Responsibilities
- Interviewing patients and/or their families
- Utilizing active listening skills
- Utilizing available resources to overcome barriers (i.e. language barriers)
- Understanding basic medication information
- Integrate and collaborate with the emergency department’s admitting clinical teams
- Operate electronic health records to obtain patient medication history information
- Contact outside medical facilities to obtain patient medication history information
Requirements
- High school diploma or equivalent required
- Three years pharmacy experience required, with both community and inpatient hospital experience preferred
- Must be proficient in math with knowledge and ability to use a computer for data entry
- Must be able to follow oral and written instructions with accuracy, efficiency, and dependability
Qualifications

- Credential in pharmacy technician registration and national pharmacy certification by Pharmacy Technician Certification Board (PTCB)
- Experience with medication history program preferred
- Community and/or ambulatory pharmacy experience preferred

Benefits
The organization offers a signing bonus to eligible candidates. The position also provides a competitive salary range of $23.81 - $40.14 per hour, with the final offer varying based on experience, skills, qualifications, and internal equity considerations. The schedule includes Monday through Friday, Saturday, with future weekend rotation.
