Responsibilities
- Aid with pharmacy referrals concerning medication access, affordability, education, adherence, optimization, and safety under pharmacist supervision.
- Handle and respond to member inquiries about medications promptly.
- Conduct medication reviews by collecting, reviewing, and documenting relevant information for clinical assessment.
- Collaborate with nurses, pharmacists, care coordinators, providers, and other interdisciplinary care team members to develop and implement member care plans and manage transitions of care.
- Enter and maintain accurate data in designated systems and ensure timely, complete, and compliant documentation.
- Create and update medication access and pharmacy resource materials for both internal teams and members.
- Maintain and monitor assigned tasks and referrals to ensure timely follow-up and resolution.
- Comply with policies, procedures, and state/federal regulations to ensure confidentiality of Protected Health Information (PHI).
- Engage in compliance and other training programs as part of employment requirements.
- Incorporate in quality improvement activities and process enhancement initiatives as assigned.
Requirements
- High school diploma or equivalent.
- Pharmacy technician registration/license required, pharmacy technician certification preferred.
- At least 1 year of pharmacy technician experience, preferably in managed care, community/retail pharmacy, specialty pharmacy, or healthcare coordination settings.
- Bilingual proficiency is preferred but not required.
- Familiarity with medical insurance terminology and processes.
- Understanding of general pharmacy terminology, medication names, dosage forms, and SIG codes.
- Reliable high-speed internet access.
Skills
- Proficiency in Microsoft Office: Outlook, Word, Excel, Teams.
- Excellent written and verbal communication skills.
- Strong organizational skills and attention to detail.
- Ability to manage multiple tasks, solve problems, work independently, and escalate issues appropriately.
- Collaborative approach with empathy and professionalism when interacting with members and providers.
- Comfortable communicating with members regarding medication lists.
Working Conditions
- Sitting for extended periods.

- Repetitive motion for typing and/or texting.
- Good manual dexterity for handling packages, paperwork, operation of motor vehicles and/or electronic devices.
- Sense acuity for seeing/read computer monitors or other electronic devices.
- Visual acuity for driving, including peripheral vision and depth perception.
- Possible exposure to travel, traffic, and/or other outdoor hazards.
- Exposure to various weather conditions, including extreme heat, cold, rain, and snow.
