Pharmacy Tech Medical History
Job Summary
The medication history pharmacy technician specialist performs medication histories for new hospital admissions. Under general supervision of the clinical pharmacist, they provide complete and accurate information for each medication, verify/updates patient allergy, adverse drug reaction, preferred pharmacy and primary care physician information in the electronic medical record, documents medication histories in the electronic health record and CPW/VigiLanz, assists the medication reconciliation pharmacist, prepares education materials, and documents medication histories.
Qualifications
- High School Graduate / GED (PHARM-Intern)
- Certified Pharmacy Technician (CPhT) or Pharmacy Technician License
Responsibilities
- Collect medication histories through patient/family interview, medication lists, outside facility records, and physician office records
- Verify/Update patient allergy, adverse drug reaction, preferred pharmacy, and primary care physician information in the electronic medical record
- Document medication histories in the electronic health record and CPW/VigiLanz in an accurate and timely manner
- Prepare education materials for the medication reconciliation pharmacist
- Absorb and document medication histories for patients identified for discharge counseling and medication reconciliation
Requirements
- Ability to work flexible hours including weekends, evenings, holidays, on-call, and additional hours as needed
