Medication History Pharmacy Tech

Responsibilities

• Counsels patients to obtain a medication history.
• Contact outside healthcare providers and pharmacies to clarify/verify patient-reported lists.
• Documents communications in the EMR.
• Accurately records current medications, supplements, vitamins, vaccines, and herbal remedies in the EMR.
• Communicates missing information or details to providers or pharmacists.
• Collaborates with provider staff to identify, clarify, and communicate missing or clinically significant patient history information.
• Records or updates medication allergies and adverse reactions in the EMR.
• Demonstrates good time management to maintain efficient workflow.

Qualifications

• Certified Pharmacy Technician (CPhT) from the national Pharmacy Technician Certification Board (PTCB).
• High School Diploma or equivalent.
• Three (3) years of experience working in a customer service capacity.
• Three (3) years of experience working as a pharmacy technician.
• Proficient pharmaceutical mathematics skills, including multiplication, ratios, and percentages.
• Formal training as a pharmacy technician from an educational institution or hospital preferred.
