Pharmacy Specialist - Specialty Infusion

Summary of Job Duties

The Pharmacy Specialist for Specialty Infusion is responsible and accountable for the provision of safe, effective, and prompt medication therapy. They work under minimal supervision to provide direct patient-centered care and integrated pharmacy operational services that optimize the patient care experience and drive organizational performance.

This position provides expertise not routinely available in the pharmacy staff. The specialist serves as a member of an interdisciplinary team representing UAMS Specialty Pharmacy Infusion Services and provides pharmacy services with a focus on safety, efficacy, and positive outcomes. The specialist coordinates clinical pharmacy services for UAMS Specialty Pharmacy Infusion Services to optimize therapeutic utilization, patient outcomes, and safety, while minimizing adverse events.

Qualifications

• Minimum Qualifications: Graduate of an ACPE-accredited College of Pharmacy with a Doctor of Pharmacy or BS Pharmacy Degree, PGY1 residency training OR 3 years of applicable experience (the dept can specify experience), Certification as a Licensed Pharmacist by the State of Arkansas or eligible for licensure.
• Preferred Qualifications: PharmD 5 years applicable experience, Experience coordinating outpatient infusion therapy

Responsibilities

• Develop and maintain a clinical pharmacy practice with the goal of optimizing safe, effective, and affordable medication therapy.
• Review infusion treatment plans for appropriateness and proper dosing based on the documented diagnosis and the objective information found in patient’s chart as well as current guidelines.
• Pharmacokinetic drug monitoring and dose adjustment.
• Cook up pre-infusion medications, labs, and vaccinations, assisting with logistics of clinic-administered vs pharmacy-administered vaccines per payor requirements.
• Coordinate protocol and order set development, formulary management, drug utilization reviews, and cost-savings initiatives.
• Coordinate care with the Prior Authorization and Financial Assistance teams to ensure timely and affordable authorization and start of infusion therapy, including both treatment and therapy plans.
• Suggest formulary alternatives, where appropriate, or document clinical need for nonformulary and/or off-label usage where appropriate.
• Ensure compliance with necessary infusion REMS requirements.
• Apply and make necessary modifications to outpatient treatment and therapy plans on a patient-specific basis.
• Aid in writing appeal letters and letters of medical necessity as needed for medication authorization purposes.
• Aid in implementing health-system pharmacy initiatives (e.g. dose rounding, biosimilar use, etc).
• Support the transition of care from outpatient infusion to Specialty Pharmacy oral/self-injectable medications.
• Provide education and counseling to professionals and patients as needed.
• Collaborate with the other clinical pharmacy specialists in Outpatient Specialty clinics on a daily basis, and serve as the liaison between the outpatient infusion services in these clinics and applicable UAMS pharmacies.
• Precept pharmacy students and residents.
• Meet key performance indicator standards for patient volume and level of care.
