About the role

The Mount Sinai Health System seeks a pharmacist to provide clinical pharmacy services, promote rational drug therapy, and support the development of new pharmacy practice programs.

Responsibilities

• Participate in daily multidisciplinary internal medicine hospitalist rounds
• Provide accurate and timely medication information to healthcare staff and patients
• Provide clinical and operational pharmacy support to pharmacy and medicine teams
• Collaborate with the Director and Pharmacy and Associate Director of Pharmacy to develop and implement clinical pharmacy programs and services
• Serve as an active participant in local and system committees in related clinical practice areas
• Contribute to the Antimicrobial Stewardship program efforts
• Contribute to the Transitions of Care services and programs
• Identify, design, and analyze drug utilization reviews to promote rational drug therapy and determine action plan based on results
• Participate in departmental and hospital quality and cost-savings initiatives
• Sustain the hospital formulary, minimizing non-formulary procurement, utilizing therapeutic interchanges and promote rational drug therapy selection
• Coordinate formulary reviews, drug use evaluations, and quality assurance programs
• Develop and implement systems and procedures to maintain an efficient and effective workflow within the pharmacy and/or assigned areas
• Address issues or services requiring intradepartmental coordination proactively
• Ensure an adequate inventory of pharmacy products and supplies to meet department demands
• Ensure the department is in compliance with all federal, state and local regulations, accrediting agencies; and Mount Sinai Heath System policies and procedures
• Contribute to managing various personnel functions such as hiring, training, performance appraisals, coaching, disciplinary actions, and terminations
• Exchange information and provide input for inter-and-intra departmental meetings
• Serve on a rotating basis, as an on-call administrator and troubleshoot issues that arise evenings, weekends, and holidays
• Maintain adequate documentation to satisfy JCAHO requirements or other regulatory agencies
• Perform other duties as assigned

Requirements

• Doctor of Pharmacy Degree required
• Current NYS Pharmacist license and registration
• ASHP-Accredited PGY-1 required, PGY-2 residency preferred
• Board certification (BCPS, BCCCP) strongly encouraged
• Hospital pharmacy and/or clinical experience; management experience preferred
