Pharmacy Intern YR3
Job Summary
The Pharmacy Intern YR3 supports the mission of the Pharmacy department by ensuring the right patient receives the right medication safely and promptly under direct supervision. Duties include understanding the drug distribution system, gaining experience in various aspects of pharmacy practice, and participating in interdisciplinary rounds.
Essential Functions
- Participate in interdisciplinary rounds.
- Work collaboratively with the treatment team to enhance patient outcomes.
- Conduct medication history interviews, first dose teaching, food-drug interaction counseling, discharge counseling, and adherence assessments.
- Create updated medication lists for patients upon discharge from hospital stays or clinic appointments.
- Counsel patients on prescription and over-the-counter medications and devices.
- Maintain medication therapies through profile reviews, interpreting laboratory values, and ensuring drug regimens align with guidelines and protocols.
Qualifications
- High School Diploma required.
- No experience in lieu of a degree accepted.
- Requires a Pharmacy Intern [State License].
