About the role

The Customer Transitions Coordinator serves as the assigned project manager for Consonus Pharmacy’s new customer onboardings, guiding senior living communities and long-term care facilities through the transition from their current pharmacy to Consonus. This role owns the end-to-end project plan for each onboarding—building detailed timelines, driving milestones and readiness activities, coordinating cross-functional teams, and serving as the primary customer-facing point of contact from sales handoff through go-live and post-start stabilization.

This position is outward-facing and project-oriented: it leads customer communication, facilitates kickoff and weekly transition calls, and manages the many moving pieces of each implementation—including intake, system build, integration/eMAR coordination, order-entry readiness, equipment logistics, and go-live readiness. The Customer Transitions Coordinator partners closely with internal and external stakeholders to deliver successful, on-time transitions that advance patient safety, operational excellence, and customer satisfaction.

Responsibilities

  • Serve as project manager for new customer onboardings, guiding facilities through the transition to Consonus Pharmacy.
  • Own the end-to-end project plan for each onboarding, including building detailed timelines and driving milestones.
  • Coordinate cross-functional teams and readiness activities.
  • Act as the primary customer-facing point of contact from sales handoff through go-live and post-start stabilization.
  • Lead customer communication and facilitate kickoff and weekly transition calls.
  • Manage implementation components such as intake, system build, integration/eMAR coordination, order-entry readiness, equipment logistics, and go-live readiness.
  • Partner with internal and external stakeholders to ensure successful, on-time transitions that enhance patient safety, operational excellence, and customer satisfaction.

Requirements

  • High school graduate or equivalent.
  • Licensed pharmacy technician with active, good-standing licensure with each state board of pharmacy where a license is held.
  • Willingness to obtain necessary licensure in additional states served by the company as required.
  • Minimum two years of experience in project coordination, customer onboarding/implementation, healthcare operations, or pharmacy operations.

Skills

  • Demonstrated ability to coordinate cross-functional teams and drive projects to on-time completion.
  • Strong communication, organizational, facilitation, and problem-solving skills.
  • Proficiency with Microsoft 365 applications, including Outlook, Teams, Excel, SharePoint, and Planner.
