Essential Duties and Responsibilities
- Reviews prescriptions for accuracy and suitability.
- Provides information and advice regarding drug interactions, side effects, dosage, and storage.
- Affirms the identity, strength, or purity of medications.
- Compounds and dispenses medications as per prescriptions.
- Maintains records such as pharmacy files, patient profiles, and inventory.
- Ensures quality, security, and proper disposal of pharmaceuticals.
- Determines and resolves medication therapy issues.
- Monitors and intervenes for drug interactions.
- Verifies medication orders for appropriateness and accuracy.
- Calculates and provides drug dose information.
- Participates in formulary management.
- Assists in medication reconciliation.
- Evaluates and reports medication errors and adverse drug reactions.
- Interprets and fills physician orders and prescriptions.
- Performs or oversees intravenous fluid compounding and manipulation.
- Maintains knowledge of and improves medication delivery systems.
- Collaborates with healthcare professionals for drug applications and characteristics.
- Analyzes prescribing trends for patient compliance and interaction prevention.
- Orders and manages pharmaceutical supplies and medical equipment.
- Prepares and reviews policies and procedures.
- Participates in QAPI for assigned departments.
- Maintains a safe work environment and complies with safety protocols.
- Maintains confidentiality of patient, resident, employee, and organizational information.
Qualifications
- Bachelor’s degree in pharmacy or Doctor of Pharmacy degree.
- Licensed to practice pharmacy in the State of Iowa.
- Strong patient service skills.
- Excellent knowledge of chemistry and drug interactions.
- Strong medical and biological terminology knowledge.
- Excellent attention to detail and balancing priorities.
