Pharmacist (PRN or Weekend Package)

Essential Duties and Responsibilities

• Reviews prescriptions for accuracy and suitability.
• Provides information and advice regarding drug interactions, side effects, dosage, and storage.
• Affirms the identity, strength, or purity of medications.
• Compounds and dispenses medications as per prescriptions.
• Maintains records such as pharmacy files, patient profiles, and inventory.
• Ensures quality, security, and proper disposal of pharmaceuticals.
• Determines and resolves medication therapy issues.
• Monitors and intervenes for drug interactions.
• Verifies medication orders for appropriateness and accuracy.
• Calculates and provides drug dose information.
• Participates in formulary management.
• Assists in medication reconciliation.
• Evaluates and reports medication errors and adverse drug reactions.
• Interprets and fills physician orders and prescriptions.
• Performs or oversees intravenous fluid compounding and manipulation.
• Maintains knowledge of and improves medication delivery systems.
• Collaborates with healthcare professionals for drug applications and characteristics.
• Analyzes prescribing trends for patient compliance and interaction prevention.
• Orders and manages pharmaceutical supplies and medical equipment.
• Prepares and reviews policies and procedures.
• Participates in QAPI for assigned departments.
• Maintains a safe work environment and complies with safety protocols.
• Maintains confidentiality of patient, resident, employee, and organizational information.

Qualifications

• Bachelor’s degree in pharmacy or Doctor of Pharmacy degree.
• Licensed to practice pharmacy in the State of Iowa.
• Strong patient service skills.
• Excellent knowledge of chemistry and drug interactions.
• Strong medical and biological terminology knowledge.
• Excellent attention to detail and balancing priorities.
