Responsibilities

  • Interprets medication orders (verbal and written) and transcribes to computerized patient medication profiles accurately.
  • Maintains accurate, complete patient medication profiles.
  • Monitors medication therapy regimens for contraindications, interactions, allergies and appropriateness of medication and dose.
  • Safely, appropriately, and cost-effectively manages medication therapies for patients according to established policies, procedures, and protocols.
  • Supervises and directs daily activities of pharmacy technicians.
  • Accurately and timely provides medication information to the professional staff.
  • Periodically audits medication administration as instructed by System Pharmacy Director.
  • Completes and documents inspections of all assigned medication storage areas at least monthly.
  • Satisfactorily demonstrates the knowledge, skills, and abilities to perform the duties outlined in this job description as well as continually demonstrates competency in performing the job duties.
  • Satisfactorily demonstrates the knowledge, skills, and abilities to perform the duties for Antibiotic Stewardship.

Qualifications

  • Licensure as a Registered Pharmacist by the Oklahoma State Board of Pharmacy required.
  • Certificate as a preceptor by the Oklahoma State Board of Pharmacy after one year of licensure preferred.
  • Education: Undergraduate pharmacy degree (Bachelor of Science) or Pharm.D preferred.
  • One (1) year experience in a hospital pharmacy preferred.
  • Must be highly proficient in the clinical skills regarding patient assessment and pharmaceutical care.
  • AHA BLS-HCP within 30 days.
