Position Summary

Captures and administers pharmaceutical care for patients. Oversees drug therapy protocols within an interdisciplinary team. Validates medication orders for accuracy and completeness. Ensures proper execution of drug information and education.

Responsibilities

• Coordinates and provides pharmaceutical care to patients.
• Manages drug therapy protocols as part of an interdisciplinary team.
• Reviews, interprets, and evaluates medication orders to identify and resolve drug-related issues.
• Provides drug information and education to medical staff, healthcare professionals, and patients.
• Documents interventions and their outcomes for quality improvement.
• Evaluates medication orders for accuracy and completeness before order entry.
• Checks for accuracy after medication preparation by technicians.

Requirements

• Bachelor's Degree in Pharmacy Required
• Doctoral Degree Preferred
• Pharmacy License in Kentucky Required
• Requires critical thinking, decisive judgment, and minimal supervision.
• Must be able to work in a stressful environment and handle emergencies.

Qualifications

• Strong communication and interpersonal skills.
• Ability to work independently and manage multiple tasks.
• Knowledge of pharmacology and clinical pharmacy practices.
• Experience with electronic health records (EHR).
