Responsibilities
- Serves as a point of contact for patients and healthcare providers to address medication-related issues.
- Explains care plan options, medication details, and potential side effects to patients and caregivers.
- Ensures accurate documentation of patient care decisions and actions using proprietary software.
- Manages medication distribution and fulfillment processes using proprietary software.
- Assesses, compiles, packages, and labels medications according to policy and legal requirements.
- Monitors and ensures compliance with policies and procedures, as well as regulatory guidelines.
- Reviews and approves prescription orders and dispensed medications to ensure accuracy and compliance.
- Works to ensure proper and compliant billing and record-keeping of medications dispensed and shipped.
- Provides timely and appropriate pharmaceutical care, assessing new medication requests and refills as needed.
- Ensures compliance with state and federal laws and regulations, including HIPAA and Fraud, Waste, and Abuse.
- Reports and addresses any incidents of Quality Assurance (QA), HIPAA violations, Fraud, Waste, and Abuse or other issues.
Qualifications
- Graduate from a pharmacy program accredited by ACPE or equivalent international program.
- Current, active license/registration in good standing with the state board of pharmacy.
- At least 2 years of experience in pharmacy distribution and packaging, with exposure to geriatrics and/or chronic care management.
- Experience or strong interest in clinical pharmacogenomics is preferred.
