Pharmacist-Oncology/Hematology - Sharp Grossmont Hospital - Day Shift - Full Time

To provide the professional, clinical and technical knowledge, and skills and expertise required for the provision of comprehensive pharmaceutical care that supports the mission, values and philosophy of Pharmacy Services and Sharp HealthCare.

Required Qualifications

• Education pursuant to licensure requirement.
• AHA Basic Life Support for Healthcare Professional (AHA BLS Healthcare)
• California Pharmacist License
• Completed a Pharmacy Practice Residency with experience in Oncology or a specialized clinical residency in Oncology and Hematology care.
• BPS Board Certified Oncology Pharmacist (BCOP)

Preferred Qualifications

• Completed a Pharmacy Practice Residency with experience in Oncology or a specialized clinical residency in Oncology and Hematology care.
• BPS Board Certified Oncology Pharmacist (BCOP)

Essential Functions

• Provides clinical advisory services.
• Participates in clinical intervention programs.
• Completes submission of electronic forms.
• Responds to and documents drug information requests.
• Reviews patient medication profiles.
• Communicates with nurses and physicians.
• Follows up on formulary compliance issues.
• Participates in Code-Blue emergencies.
• Consistently participate in financial pillar/cost savings measures.
• Provides clinical consult services.
• Attains consult service goals.
• Participates in Oncology & Hematology care team meetings.
• Participates in Oncology & Hematology based and hospital-wide committees.
• Operational Services (working with internal customers).
• Intercepts and troubleshoots problems.
• Evaluates ongoing workflow issues.
• Attends and participates in staff meetings.
• Reads work related email.
• Customer Service Interdepartmental Relations (working with external customers).
• Answers phone in timely, friendly manner.
• Fosters positive relationships.
• Responds to requests.
