Responsibilities Monitor inpatient medication orders and verify accuracy, under pharmacist supervision.Ensure 24/7 medication distribution accuracy during night shifts.Maintain strict control and documentation of controlled substances.Adhere to hospital policies, accreditation standards, and best practices.Counsel patients and caregivers on medication use, dosing, and safety.Collaborate with medical and nursing staff to optimize therapy plans.Dependable, data-driven decision making to meet inventory and usage targets.Participate in quality and safety initiatives to reduce medication errors.Document all interventions and clinical notes in the EHR system.Lead or participate in pipeline and forecasting for medication needs.Support ongoing professional development through self-learning and training.Act as a point-of-contact for medication-related inquiries from staff. Experience/Qualifications Pharmacist license active and in good standing for state of practice.Inpatient hospital pharmacy experience, including ward rounds and med distribution.Strong knowledge of controlled substances handling and documentation.Night shift competency with 7 on/7 off scheduling and patient safety focus.Proficient in medication order verification and IV/handling protocols.EHR/documentation accuracy and timely clinical notes entry.Quality, safety initiative participation; implement error-reduction measures.Counseling patients and caregivers on dosing, administration, and safety.Collaboration with medical/nursing staff to optimize therapy plans.Inventory forecasting and pipeline planning for medication needs.Data-driven decision making with accountability for personal targets.Open to ongoing professional development and structured training.Ability to serve as a point-of-contact for staff medication inquiries.
