Required Qualifications

• Education pursuant to licensure requirement.
• 3 Years experience in a hospital pharmacy practice;
• Or 1 Year completion of PGY1 Pharmacy Residency.
• CALIFORNIA PHARMACIST LICENSE - CA STATE BOARD OF PHARMACY - REQUIRED
• AHA ACLS Certification (Advanced Cardiac Life Support) - AMERICAN HEART ASSOCIATION - REQUIRED
• AHA BLS for Healthcare Professional (AHA BLS Healthcare) - AMERICAN HEART ASSOCIATION - REQUIRED

Preferred Qualifications

• 2 Years completion of PGY-2 Pharmacy Residency.

Essential Functions

• Verification and review of medication orders
• Makes sure medication orders are correct
• Makes sure therapy is safe
• Makes sure inpatient drug formulary is followed
• Provides counseling to patients
• Assesses patient understanding
• Ensures drug regimen is right
• Monitors drug regimen
• Reviews adverse drug reactions
• Reviews for disease progression
• Communicates with health care providers
• Assesses patient’s progress
• Participates in clinical intervention program
• Communicates with nurses and physicians
• Follows up on formulary compliance issues
• Documents interventions
• Participates in scheduled rounds
• Oversees Pharmacy Resident and/or PharmD student participation on rounds

• Addresses patient specific drug information questions
