Responsibilities

  • Prepare and dispense medication orders per physician request according to established policies, procedures, and protocols.
  • Ensure safe, appropriate, and cost-effective medication therapies for patients under the direction of leadership.
  • Provide input to leadership for the review or creation of departmental policies, procedures, and protocols.
  • Supervise and direct pharmacy support personnel in accordance with NYS Board of Pharmacy regulations and departmental guidance.
  • Verify daily activities of pharmacy technicians, addressing errors in a constructive, non-punitive manner.
  • Maintain a professional appearance and wear identification at all times.
  • Prioritize and complete work within specified times to ensure patient safety and satisfaction.
  • Adapt positively to frequent interruptions and changes in workload or schedule.
  • Collaborate with network staff, including pharmacy management, providers, and leadership.
  • Address problems using appropriate tools and techniques for identification and resolution.
  • Demonstrate receptiveness to changes that enhance pharmacy services and proactively engage in improvement efforts.
  • Perform other assigned tasks and identify/resolve immediate needs, escalating issues when necessary.

Pharmacist II (Inpatient)

  • Perform and supervise sterile compounding according to Hospital policy and USP 797 standards.
  • Perform and supervise hazardous sterile compounding according to Hospital policy and USP 800 standards.
  • Review and verify prescriber’s orders for appropriateness, accuracy, and adherence to hospital standards and formulary.
  • Interact with practitioners, nursing, and ancillary staff on topics impacting patient care, policy, and formulary adherence.
  • Order, receive, manage, and secure medication and ancillary product inventories, including controlled substances, in compliance with regulations and policies.
  • Ensure accurate and complete dispensing of medications in accordance with departmental, accreditation, and regulatory standards.
  • Distribute medications within healthcare clinics, operating rooms, and other clinical service areas.
  • Interact with patients to obtain medication therapy histories and provide counseling and instruction.
  • Participate in the development of BHN policy and procedures, as well as local entity and BHN committees.
  • Engage in staff development, education, and support.

Pharmacist II (Oncology)

  • Perform all duties of Pharmacist II (Inpatient).
  • Work within the Cancer Treatment Center, performing compounding of sterile preparations (both hazardous and non-hazardous) in accordance with USP 797, USP 800, accreditation standards, and state regulations.
  • Provide direct oversight of technicians in sterile compounding and educate staff on regulations.
  • Offer clinical support to the Cancer Treatment Center team, including providers, nurses, and ancillary staff.
  • Collaborate with Pharmacy Leadership and CTC leadership on initiatives, projects, and day-to-day operations.
  • Provide clinical support to patients, including drug review and education.

Pharmacist II (Outpatient)

  • Prepare and dispense prescriptions using clinical judgment and available resources, following safe practices and established policies.
  • Review prescriptions for appropriateness, accuracy, and safe labeling.
  • Maintain accurate patient profiles and pharmacy records in compliance with Network policies, state, and federal regulations.
  • Maintain accurate inventory of controlled substances, adhering to Network policies, NYS BNE, and Federal DEA requirements.
  • Interact with providers and healthcare professionals to monitor, review, and evaluate medication effectiveness.
  • Educate and counsel patients on medications, side effects, drug interactions, and proper use of OTC medications and supplements.
  • Supervise pharmacy technicians according to network policies and state regulations.
  • Ensure compliance with NYS laws, regulations, and accreditation guidelines.
  • Resolve problem prescriptions independently and in a timely manner.
  • Communicate professionally with hospital staff and patients, checking all products requiring pharmacist approval prior to dispensing.
  • Participate in departmental Q/A processes, including reporting and following up on medication incidents and errors.
  • Identify and address barriers to patient discharge, including financial barriers, and participate in interdisciplinary teams.
  • Provide immunizations to patients according to CDC recommendations and Network procedures.
  • For certified pharmacists (e.g., Diabetes Education, Medication Therapy Management), provide services to patients as requested by Pharmacy Leadership.
  • Assist Pharmacy leadership in developing programs for patient care within the area of certification.

Pharmacist II (Specialty)

  • Support timely access to specialty therapies through prior authorizations, benefits investigations, and appeal processes.
  • Provide disease-specific and medication-related education to support patient understanding, engagement, and adherence.
  • Conduct comprehensive medication therapy reviews to assess safety, efficacy, and optimal use of specialty medications.
  • Review, approve, and enter medication orders in the computerized pharmacy system, ensuring accurate patient-specific parameters.
  • Dispense ordered medications in the correct quantity, dosage, and form at the proper time.
  • Review and approve formulations for compounded medications and check final products prior to dispensing.
  • Maintain documentation and records for drug purchasing, storage, preparation, and distribution in compliance with policies and regulations.
  • Ensure proper handling, storage, and delivery of prescription pharmaceuticals, including cold chain requirements.
  • Support enrollment and adherence monitoring for patients referred for Specialty Care Management.
  • Collaborate with prescribers, nurses, case managers, and payers to ensure smooth initiation and continuity of therapy.
  • Monitor clinical outcomes, adverse events, and adherence patterns to evaluate therapeutic outcomes.
  • Document all clinical activities, interventions, and outcomes for quality improvement and regulatory compliance.
  • Ensure specialty pharmacy operations meet URAC/ACHC standards, as well as manufacturer and payer requirements.
  • Maintain accurate documentation for audits and accreditation readiness.
  • Contribute to the development of clinical protocols, therapeutic management pathways, and formulary strategies.
  • Lead or support initiatives to improve workflow efficiency, patient experience, and program growth.
  • Provide in-service education to pharmacy staff, students, nurses, physicians, and other healthcare practitioners.
  • Maintain professional and specialized competency through continued education.

Requirements

  • 5-year degree in Pharmacy or PharmD, required.
  • Must maintain all requirements, including continuing education, as mandated by the New York State Board of Pharmacy.
  • 3 years of experience as a practicing Pharmacist, required.
