About the Role

This position provides services at the TCRHCC main campus. The Pharmacist is responsible for delivering comprehensive pharmacy and medication management services to patients, ensuring consistent service in accordance with TCRHCC policies. Direct patient care is provided to neonatal, pediatric, adolescent, adult, geriatric, oncology, and obstetric patients. The role requires working rotational shifts, including days, evenings, nights, weekends, and holidays.

Career advancement opportunities exist through tiered levels from Pharmacist to Advanced Practice Pharmacist, determined by pharmacy experience. The Advanced Practice Pharmacist serves as a clinical pharmacist for inpatient and outpatient departments, with additional responsibilities for specialized pharmacy-based clinics, policy administration, and program direction. The Advanced Practice Pharmacist II also manages clinic operations and supervises professional and non-professional staff.

Responsibilities

  • Conducts authorized outpatient pharmacy clinics for examination, diagnosis, and treatment of specialized clinic patients, including obtaining health histories, adjusting treatment plans, and monitoring outcomes.
  • Reviews inpatient and outpatient medical records for safety, appropriateness, and effectiveness of drug therapy, considering patient-specific factors such as age and organ function.
  • Consults with medical and nursing staff to evaluate medication use and collaborates on patient care.
  • Reviews, assesses, and inputs chemotherapy treatment plans for oncology patients, verifying medication orders for accuracy, dosage, and safe administration.
  • Counsels patients, families, or representatives on safe and effective medication use, including compliance.
  • Participates in pandemic and disaster drills, as well as actual disaster response events.
  • Develops hospital staff’s pharmacological knowledge through training programs for pharmacy staff, students, interns, externs, residents, and healthcare professionals.
  • Maintains pharmacological expertise by attending educational workshops, reviewing professional publications, and participating in continuing education.
  • Provides pharmacological information to healthcare professionals, including drug properties related to absorption, metabolism, and excretion across all age groups.
  • In the inpatient pharmacy setting:
    • Attends multi-disciplinary rounds to provide input on therapeutic plans and adjust recommendations as needed.
    • Responds to Code Blue and Level 1 traumas, assisting with dose calculations, infusion rates, and preparation of emergent medications.
    • Plans, reviews, and maintains emergency drugs for critical situations.
    • Documents clinical activities, updates problem lists, and maintains medication allergy information in medical records.
    • Provides pharmaceutical services focused on therapeutic drug monitoring.
    • Analyzes medication regimens to ensure the most effective, least toxic, and economical treatment plans.
  • Ensures proper use of personal protective equipment (PPE), including masks, gloves, gowns, respirators, and face shields, and follows CDC guidelines for donning, doffing, and disposal.
  • Completes training for cleaning and decontamination of surfaces contaminated by communicable diseases.
  • Gains competency in utilizing pharmacy software systems.
  • Supervises and guides pharmacy technicians in delivering pharmacy services.
  • Meets key performance indicators (KPIs) and performs duties efficiently.
  • Identifies and reports medication errors or adverse drug reactions as per TCRHCC policies.
  • Evaluates areas for improvement in pharmacy or hospital practices to enhance patient safety and operational efficiency.
  • Selects, procures, prepares, and dispenses medications that meet quality standards for potency, purity, and effectiveness.
  • Evaluates prescriptions for appropriateness based on patient age, weight, and gender.
  • Demonstrates knowledge and competency in hazardous medication management, including safe handling and preparation of chemotherapeutic agents and other specialized medications.

Requirements

  • Education: Doctor of Pharmacy or Bachelor of Pharmacy degree from a school recognized by the American Council of Pharmaceutical Education.
  • Certification:
    • Current, valid, and unrestricted license to practice pharmacy in a U.S. state, territory, or the District of Columbia.
    • Current Basic Life Support (BLS) certification from the American Heart Association, maintained throughout employment.
  • Experience:
    • Pharmacist I: No experience required for entry-level positions.
    • Pharmacist II: Minimum of one (1) year of experience as a Registered Pharmacist.
  • Skills and Abilities:
    • High ethical standards with no history of complaints or substance abuse.
    • Reliable, dependable, and punctual with minimal absences.
    • Positive professional attitude and strong interpersonal communication skills.
    • Knowledge of legal dispensing practices, federal pharmacy regulations, and therapeutic incompatibilities.
    • Knowledge of sterile (including USP 800 regulations) and non-sterile compounding procedures.
    • Understanding of drug-drug, drug-disease state, and drug-allergy incompatibilities.
    • Proficiency in pharmacology, therapeutics of common disease states, pathophysiology, and pharmacokinetics.
    • Successful completion of job interviews, background checks, reference checks, and fingerprint clearance.
    • Submission of accurate and complete employment-related documents, free of false or misleading information.

  • Physical Demands:
    • Frequent sitting, standing, walking, twisting, and maintaining balance.
