Qualifications

Education: Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Licensure: Full - current and unrestricted license to practice pharmacy in a State - Territory - Commonwealth of the United States (i.e. - Puerto Rico) - or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full - current - and unrestricted licensure. A pharmacist who has - or has ever had - any license(s) revoked - suspended - denied - restricted - limited - or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005 - Part II - Chapter 3 - section B - paragraph 16. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).

Responsibilities

  • Works with provider stakeholders to develop database reports to support clinical decisions related to patient assessment and care.
  • Identifies and writes programs in Structured Query Language (SQL) code where necessary to monitor - maintain - or initiate procedures.
  • Establishes protocols for evaluating ordering patterns.
  • Serves as a resource regarding VHA data warehouse systems management information tools and provides analytic support for relevant health care teams.
  • Incorporates Clinical Patient Record System (CPRS) decision.
  • Captivates appropriate communication - marketing - and implementation strategies for the integration of clinical analytics/data mining results.
  • Validates the structure and function of provider and team-specific data reports.
  • Ensures the accuracy of information delivered for assigned program areas (e.g. - drug file - prescription costs - clinical profiles).
  • Develops communication and marketing strategies (including Intranet - SharePoint tools - provider group and one-on-one educational opportunities) to support successful stakeholders buy-in and application.
  • Maintains program outcomes - including patient safety - cost avoidance - and healthcare resource utilization.
  • Designs - develops and delivers patient and population specific analytic programs to achieve clinical outcomes and institutional performance measures.
  • Continually monitors new literature - evidence and pricing changes to improve care and cost-containment.
  • Provides feedback - education - and recommendations to providers to improve quality of patient care and healthcare resource utilization.
  • Solves issues related to data mining and data delivery services.
  • Routinely collaborates with Region 01 and the PVAHCS Office of Information and Technology (OI&T) in addressing service-related issues that prevent system functionality.
  • Develops patient targeted education that will essentially enlist the patient to prompt providers to adhere to contract and performance measure goals as appropriate.
  • Performs other duties as assigned.

Skills

  • Ability to communicate orally and in writing to persuade and influence clinical and management decisions.
  • Expert understanding of regulatory and quality standards for their program.
  • Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy.
  • Advanced skill in monitoring and assessing the outcome of drug therapies - including physical assessment and interpretation of laboratory and other diagnostic parameters.
  • Ability to solve problems - coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise.

Full Time Monday-Friday 7:30 a.m- 4:30 p.m. variable based on the needs of the facility and service line.
