About the Role

Carilion Clinic is a six-hospital health system located in beautiful Southwest Virginia. Carilion's flagship hospital, Carilion Roanoke Memorial Hospital, is a 763-bed academic medical center, level I trauma center, tertiary/quaternary referral center, and Certified Primary Stroke center that has consistently been ranked by US News as a Best Hospital in the state of Virginia. It is also affiliated with the Virginia Tech Carilion School of Medicine and has numerous post-graduate training programs, including multiple medical and surgical residencies and fellowships.

Responsibilities

  • Optimize patients' medication regimens throughout the continuum of care
  • Address a variety of pharmacokinetic medication needs with autonomy through the use of multiple P&T approved pharmacy protocols
  • Work every other weekend and holidays as they fall
  • Assist with precepting residents within several of the decentralized zones
  • Participate in resident learning experiences
  • Various other clinical experiences such as committee participation, process improvement, and research

Requirements

  • Graduate of a College of Pharmacy accredited by the Accreditation Council for Pharmacy Education (ACPE)
  • Pharmacy Practice Residency or equivalent Hospital experience is preferred
  • License to practice Pharmacy in the Commonwealth of Virginia required within 90 days of hire
  • Must possess good leadership, communication, interpersonal and teamwork skills
  • Must be able to effectively budget time, work efficiently and accurately, and follow procedures with minimum supervision
  • Excellent critical thinking and analytical skills
  • Knowledge of general office equipment and procedures including computer and keyboarding skills

Qualifications

  • Must be a graduate of a College of Pharmacy accredited by the Accreditation Council for Pharmacy Education (ACPE)
  • Must possess a current Virginia pharmacist license or be eligible for licensure

Skills

Active member of interdisciplinary care teams

Advise on the most efficacious treatments and participate in pharmacotherapeutic decision making

Provide drug information and evidence-based medicine

Consult and provide healthcare providers advice on medications

Respond to patient emergencies

Review and verify medication orders for accuracy

Ensure preparation of sterile/nonsterile compounding according to Standard Operating Procedures

Ensure delivery of medications in a timely manner

Provide education and training for healthcare providers (pharmacy, nursing and medical), students, residents, patients, and patient families

Support clinical pharmacy initiatives of the institution

Maintain compliance with statutes, laws, regulations, standards and policies governing the practice of pharmacy
