Pharmacist Clinical - Institutional

About the role

This is a full-time variable shift Pharmacist position at SSM St. Mary's Hospital in Madison, Wisconsin, working a total of 36 hours per week. Shifts are 8 hours and include a combination of days and evenings with occasional overnight coverage. Day shifts start as early as 6:30 a.m. and evening shifts end at 11:00 p.m. The schedule includes an every third weekend rotation.

A one-time sign-on bonus (less taxes) may be available to qualified individuals with a 2-year service commitment.

Responsibilities

• Ensures safety and appropriateness of medication therapy through order verification, dispensing, home medication reconciliation, chart review, patient counseling, lab value monitoring, and dose/therapy adjustment as applicable. Documents medication-related plans and interventions in the electronic medical record.
• Provides clinical consultation delivering best-practice, evidence-based, and complete information to practitioners. Makes recommendations and encourages safe and cost-effective pharmacotherapy for patients through multi-disciplinary collaboration.
• Oversees the medication dispensing process to ensure accurate and timely delivery of optimal pharmacotherapy to patients, including supervision of pharmacy technicians and/or interns.
• Precepts pharmacy students and residents, as applicable.
• Works in a constant state of alertness and safe manner.
• Performs other duties as assigned.

Requirements

• Pharmacy degree (Pharm.D or Bachelor of Pharmacy) from an accredited school of pharmacy.
• No prior experience required.
• Basic Life Support Healthcare Provider (BLS HCP) certification required within 90 days of employment. Other resuscitation certifications may be required dependent on the department.
• Active pharmacist license in the state of work location (Illinois, Missouri, Oklahoma, or Wisconsin).

Physical Requirements

• Frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs.
• Frequent sitting, standing, walking, reaching, and repetitive foot/leg and hand/arm movements.
• Frequent use of vision and depth perception for distances near (20 inches or less) and far (20 feet or more) and to identify and distinguish colors.
• Frequent use of hearing and speech to share information through oral communication. Ability to hear alarms and malfunctioning machinery.
• Frequent keyboard use/data entry.
• Occasional bending, stooping, kneeling, squatting, twisting, and gripping.
• Occasional lifting/carrying and pushing/pulling objects weighing 25-50 lbs.
• Rare climbing.
