About the role
This is a full-time variable shift Pharmacist position at SSM St. Mary's Hospital in Madison, Wisconsin, working a total of 36 hours per week. Shifts are 8 hours and include a combination of days and evenings with occasional overnight coverage. Day shifts start as early as 6:30 a.m. and evening shifts end at 11:00 p.m. The schedule includes an every third weekend rotation.
A one-time sign-on bonus (less taxes) may be available to qualified individuals with a 2-year service commitment.
Responsibilities
- Ensures safety and appropriateness of medication therapy through order verification, dispensing, home medication reconciliation, chart review, patient counseling, lab value monitoring, and dose/therapy adjustment as applicable. Documents medication-related plans and interventions in the electronic medical record.
- Provides clinical consultation delivering best-practice, evidence-based, and complete information to practitioners. Makes recommendations and encourages safe and cost-effective pharmacotherapy for patients through multi-disciplinary collaboration.
- Oversees the medication dispensing process to ensure accurate and timely delivery of optimal pharmacotherapy to patients, including supervision of pharmacy technicians and/or interns.
- Precepts pharmacy students and residents, as applicable.
- Works in a constant state of alertness and safe manner.
- Performs other duties as assigned.
Requirements
- Pharmacy degree (Pharm.D or Bachelor of Pharmacy) from an accredited school of pharmacy.
- No prior experience required.
- Basic Life Support Healthcare Provider (BLS HCP) certification required within 90 days of employment. Other resuscitation certifications may be required dependent on the department.
- Active pharmacist license in the state of work location (Illinois, Missouri, Oklahoma, or Wisconsin).
Physical Requirements
- Frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs.
- Frequent sitting, standing, walking, reaching, and repetitive foot/leg and hand/arm movements.
- Frequent use of vision and depth perception for distances near (20 inches or less) and far (20 feet or more) and to identify and distinguish colors.
- Frequent use of hearing and speech to share information through oral communication. Ability to hear alarms and malfunctioning machinery.
- Frequent keyboard use/data entry.
- Occasional bending, stooping, kneeling, squatting, twisting, and gripping.
- Occasional lifting/carrying and pushing/pulling objects weighing 25-50 lbs.
- Rare climbing.
