Job Summary
Reviews prescriptions to ensure accuracy, determine necessary ingredients, and assess their suitability. Provides information and advice regarding drug interactions, side effects, dosage, and proper medication storage. Counsels patients on medication administration, possible side effects, expected responses, and storage. Monitors patients' health and progress in response to drug therapy.
Responsibilities
- Reviews, interprets, and dispenses both prescription and non-prescription medication to patients.
- Takes an active role in addressing prescription issues and advising providers on medication selection, dosage, interactions, and side effects.
- Counsels patients on medication prescription, including proper administration, possible side effects, expected response, and storage.
- Maintains oversight and clinical supervision for pharmacy staff and support personnel.
- Utilizes age-related patient information to assess pharmaceutical needs and provide care to neonatal, pediatric, adolescent, adult, and geriatric patients.
- Fulfills responsibilities within various departments including acute care surgery, neurology, cardiology, pediatrics, oncology, infusion services, intensive care, general medicine, dialysis, transplant, antimicrobial stewardship, and medication reconciliation.
- Might involve direct patient interaction, interdisciplinary team rounding, chart reviews, customer or patient counseling, collecting accurate medication lists, and overseeing clinical functions such as checking and dispensing medications and managing the IV room.
Qualifications
- In a retail/community pharmacy setting: Graduated from an accredited School of Pharmacy; thorough knowledge of drugs and their actions; knowledge of state and local regulations.
- In a medical center or institutional setting: Graduated from an accredited School of Pharmacy; thorough knowledge of drugs and their actions; knowledge of state and local regulations. Hospital experience or residency preferred.
