About the role
The Pharmacist will be responsible for preparing, dispensing, and managing medications in a hospital setting, ensuring safe and effective use of pharmaceutical drugs for inpatient and outpatient care across all age categories. They will collaborate with medical providers, nurses, and other healthcare professionals to optimize patient outcomes.
Responsibilities
- Complete accurate order review, preparation, and dispense of medications
- Perform medication therapy monitoring to ensure appropriate medication regimens
- Complete admission and discharge medication reconciliation
- Collaborate with medical providers on the selection, dosage, interactions, and side effects of medication
- Avoid non-compliance with hospital policies, legal, and regulatory standards related to the practice of pharmacy
- Ensure compliance with department and institution policies, rules, and regulations promulgated by the Board of Pharmacy, and other applicable regulatory bodies
Requirements
- Required: Bachelor’s Degree in Pharmacy
- PREFERRED: Doctorate in Pharmacy
- Licensure: Registered as a Pharmacist with the State of Wisconsin Department of Safety and Professional Services
- Current BLS certification at hire and ACLS within 6 months of hire.
- Computer Competency: Basic Computer Skills are expected for this position. Access email, reference internal websites, access electronic health records, and use online learning tools to remain current with continuing education requirements.
- Microsoft Office Suite Skills: Beginning Skills – Able to perform basic things such as opening a document, change font, save a document, write a letter.
