Responsibilities

  • Adheres to organization policies and procedures.
  • Collaborates with nursing, customer service, and distribution to facilitate new referrals and admissions.
  • Reviews and verifies new patient orders and ensures consistency between the order form and the patient's information.
  • Collaborates with physicians and healthcare professionals to plan and implement patient care, ensuring safe and effective drug therapy.
  • Conducts assessments of patients, drugs, and diseases to identify potential drug-related issues and needs.
  • Maintains thorough documentation of clinical activities and communications with patients, physicians, and other healthcare professionals.
  • Prepares progress reports for referring physicians on a regular basis.
  • Serves as a designated person for sterile compounding activities, if applicable.
  • Maintains pharmacy records according to applicable laws and regulations.
  • Participates in continuing education and maintains clinical skills and knowledge of pharmacotherapy.
  • Attends designated department meetings and performs other tasks or special projects as requested.

Qualifications

  • A Bachelor of Science in Pharmacy (RPh) or Doctor of Pharmacy (PharmD) from an accredited college of Pharmacy.
  • Currently licensed as a registered pharmacist in the state of employment.
  • Two years recent experience in an acute-care setting or one year recent experience in home infusion therapy.
  • Experience and strong knowledge in intravenous admixture services and intravenous therapy, if applicable.
  • Ability to evaluate and monitor drug therapy collaboratively with physicians, nurses, and other healthcare professionals.
  • Effective communication skills, both verbal and written, with patients, clients, management, and other employees.
  • Problem-solving and supervisory abilities, including the capability to manage technical and other support staff.
  • Flexibility with work schedule.
  • Good organizational and time-management skills.
