Responsibilities
- Serves in a professional capacity on unit-based decentralized teams when applicable.
- Makes sure patients get the right medicines and dosages.
- Monitors drug therapy regimens for potential issues like drug interactions and side effects.
- Works with doctors and other healthcare providers to make sure patients take the right medicines.
- Provides drug information to patients and staff.
- Reads and interprets patient charts to help with drug therapy.
- Documents clinical activities and interventions.
- Consults on drug therapy and suggests cost-effective alternatives.
- Reviews and interprets culture and susceptibility data for antibiotics.
- Ensures medications meet quality standards and are given correctly.
- Initiates and processes medication event and adverse drug reaction reports.
- Assists with data collection and testing for quality improvement.
Requirements
- Prior inpatient experience preferred.
- Pharmacy RPH - Registered Pharmacist license upon hire.
Qualifications
- PhD PharmD degree preferred.
- Bachelor's Degree BS Pharmacy from an ACPE-accredited school of pharmacy.
- 1 to 3 years experience as a hospital Pharmacist or PGY1 pharmacy residency.
