Responsibilities

  • Serves in a professional capacity on unit-based decentralized teams when applicable.
  • Makes sure patients get the right medicines and dosages.
  • Monitors drug therapy regimens for potential issues like drug interactions and side effects.
  • Works with doctors and other healthcare providers to make sure patients take the right medicines.
  • Provides drug information to patients and staff.
  • Reads and interprets patient charts to help with drug therapy.
  • Documents clinical activities and interventions.
  • Consults on drug therapy and suggests cost-effective alternatives.
  • Reviews and interprets culture and susceptibility data for antibiotics.
  • Ensures medications meet quality standards and are given correctly.
  • Initiates and processes medication event and adverse drug reaction reports.
  • Assists with data collection and testing for quality improvement.

Requirements

  • Prior inpatient experience preferred.
  • Pharmacy RPH - Registered Pharmacist license upon hire.

Qualifications

  • PhD PharmD degree preferred.
  • Bachelor's Degree BS Pharmacy from an ACPE-accredited school of pharmacy.
  • 1 to 3 years experience as a hospital Pharmacist or PGY1 pharmacy residency.
