Pharmacist
About the role
Provide comprehensive pharmaceutical care for the PACE (Program of All-Inclusive Care for the Elderly) population, supporting adults 55+ in maintaining independence and aging safely.
Review medication orders to identify, prevent, and resolve drug-related problems while recommending appropriate formulary alternatives to optimize patient outcomes.
Accurately compound and dispense medications, ensuring safe and timely delivery for a medically complex geriatric population.
Educate and counsel patients, caregivers, and interdisciplinary healthcare teams on medication therapies, adherence, and disease management.
Collaborate with physicians and care teams while supervising pharmacy support staff and assisting with training/precepting pharmacy students as needed.
Responsibilities

Requirements
- Licensure / Certification / Registration: Pharmacist obtained prior to hire date or job transfer date required.
- Education: Bachelor's or advanced degree in Pharmacy required.
Preferences
- Previous pharmacist experience in a PACE (Program of All-Inclusive Care for the Elderly) setting preferred.
- Experience working with adults aged 55+ and geriatric populations strongly preferred.
- Prior long-term care (LTC) pharmacy experience preferred.
