PH RN Case Manager- CMS

The Children's Medical Services program is seeking a highly qualified candidate to fill the full-time, salaried position of PH RN Case Manager, based in Waycross, GA.

Responsibilities

• Provide comprehensive support to families of children with special healthcare needs under general supervision, following established policies, procedures, and protocols.
• Develop and manage individualized care plans, coordinate ongoing care, and facilitate transition planning starting at age 12.
• Actively engage and retain families in the program while promoting CMS services throughout the district.
• Build and maintain relationships with community partners and participate in outreach activities.
• Manage enrollments, conduct six-month and annual reviews, and ensure continuity of care.
• Review medical records, approve billing and documentation for other nurses, and assist with phone inquiries as needed.

Success in this role requires strong organizational skills, effective communication, and a commitment to improving outcomes for families with special healthcare needs.

Requirements

• Associate degree in nursing from an accredited college or university.
• Completion of a PHN course within one (1) year of hire.
• Current Georgia Registered Professional Nurse license.
• CPR certification.

Preferred Qualifications

• Experience working with infants or young children.
• Bachelor’s degree in nursing (BSN).
• One year minimum experience as a registered professional nurse.
• Bilingual proficiency in Spanish.

Educational credentials recognized by the Council for Higher Education Accreditation (CHEA) and/or the US Department of Education (DOE) will be considered.
