About the role

To positively impact patients and their families, we seek individuals who can deliver exceptional home health, palliative, and hospice care. Our care teams serve as way finders and way makers by creating individualized care plans and experiences, wherever needed.

Responsibilities

• Deliver exceptional home health, palliative, and hospice care
• Create individualized care plans and experiences for patients and their families
• Volunteer as a Certified Pet Therapy Dog to bring joy to patients' lives
• Attend volunteer orientation and complete certification as a pet therapy dog
• Pass background checks, drug and fingerprint screenings, and TB tests
• Ensure pets are up to date on vaccinations

Qualifications

• Strong interpersonal skills and ability to connect with patients and their families
• Experience in healthcare or a related field preferred
• Ability to work independently and as part of a team
• Passionate about providing compassionate care
