Core Responsibilities
- Provide consistent, warm companionship as a trusted presence in the client's daily life
- Support clients with activities of daily living including dressing, grooming, hygiene reminders, meal support, and light mobility assistance
- Encourage socialization, meaningful activity, and healthy daily routines
- Accompany clients on walks, outings, errands, appointments, and community activities
- Support clients in maintaining independence and personal dignity at every interaction
- Prepare meals following the client's stated preferences and any dietary guidance shared by the family or medical professionals
- Assist with grocery shopping, laundry, linens, and household organization
- Support light household tasks that help the home feel safe, calm, and manageable
- Provide basic technology assistance — phone, tablet, TV, devices — to help clients stay connected
- Help coordinate household vendors and service appointments when needed
- Provide transportation using the client's vehicle whenever possible
- Accompany clients to appointments, provide escort assistance, and take notes for the client's personal reference when authorized
- Assist with appointment scheduling and follow-up coordination
- Communicate appointment outcomes and observations to the VP of Operations as directed
- Observe and document non-clinical changes in the client's condition, environment, mood, or routine using plain, factual language
- Report any concerns promptly to the VP of Operations. Do not attempt to assess or diagnose
- Communicate with professionalism and discretion in all client and family interactions
- Complete an accurate visit note at the end of every visit, same day, without exception
- Document services delivered, time in and out, and any observations or concerns
- Maintain full client confidentiality in all communications and records
- Follow all infection control, safety, and emergency protocols
- Complete required PSSA competency training and ongoing annual job-related training
- Adhere to all Tennessee Living policies, procedures, and PSSA requirements
Job Requirements
- 18 years of age or older (PSSA requirement)
- Language skills sufficient to read instructions, maintain written visit notes, and communicate clearly with clients and families
- Genuine compassion for older adults and a patient, respectful approach
- Reliable, punctual, self-directed and able to work independently in private home settings
- Valid Tennessee driver's license, reliable personal transportation, and a clean driving record
- Must pass criminal background check, TN Abuse Registry check, and TN Sexual Offender Registry check prior to first client contact
- Must complete all PSSA required competency training before providing direct services
Physical Requirements
- Ability to assist with light mobility support and positional assistance
- Ability to lift up to 25 pounds safely
- Ability to stand, bend, and move throughout client homes
- Ability to accompany clients on outings, walks, and appointments
