About the role
Permanency Caseworker (SWAN)
Responsibilities
- Work closely with children, families, and referral sources to build and execute comprehensive permanency plans.
- Provide the full range of SWAN service units, including:
- Child Profiles and Child Preparation Services
- Child-Specific Recruitment
- Family Profiles and Placement
- Finalization support
- In-home visits, case advocacy, and ongoing family support
Requirements
- Bachelor's degree in social work, counseling, or a related field
- 1–2 years of related experience and/or internship (preferred, not required)
- Experience working with trauma-impacted individuals and families
- Strong relationship-building and one-on-one communication skills
- Excellent organizational and computer skills
- Flexibility to work evenings, weekends, or non-traditional hours as needed
- Ability to adapt in a dynamic, community-based environment
Services
Services are delivered in home and community settings. Both full-time (40 hrs/week) and part-time positions are available.
Compensation & Benefits
- Salary: $38,000–$42,000 annually (for full-time)
- Retention bonus starting after 1 year
