Permanency Caseworker (SWAN)

Responsibilities

• Work closely with children, families, and referral sources to build and execute comprehensive permanency plans.
• Provide the full range of SWAN service units, including:
• Child Profiles and Child Preparation Services
• Child-Specific Recruitment
• Family Profiles and Placement
• Finalization support
• In-home visits, case advocacy, and ongoing family support

Requirements

• Bachelor's degree in social work, counseling, or a related field
• 1–2 years of related experience and/or internship (preferred, not required)
• Experience working with trauma-impacted individuals and families
• Strong relationship-building and one-on-one communication skills
• Excellent organizational and computer skills
• Flexibility to work evenings, weekends, or non-traditional hours as needed
• Ability to adapt in a dynamic, community-based environment

Services

Services are delivered in home and community settings. Both full-time (40 hrs/week) and part-time positions are available.

Compensation & Benefits

• Salary: $38,000–$42,000 annually (for full-time)
• Retention bonus starting after 1 year
