Performance Improvement Coordinator- MA or LNA
About the role
Key role in supporting value-based care efforts within family practice. This cross-functional position focuses on improving patient outcomes, closing care gaps, and ensuring compliance with payer incentive programs (e.g., Medicare Advantage, ACOs, commercial) as well as coordinating patient care within GCFH. Collaborate in team-based care within a Patient-Centered Medical Home (PCMH) to deliver coordinated, comprehensive, and personalized care that supports patient wellness and improves health outcomes. The coordinator uses data analytics and patient outreach to proactively manage patient populations and support quality metrics that align with financial incentives and regulatory goals.
Requirements
- Certification per American Association of Medical Assistants preferred (AHA) or LNA License
- BLS certification
- Proficient computer skills
- Experience working with an electronic health record preferred
