PER VISIT PHYSICAL THERAPIST HHA

Responsibilities

• Counsels clients on prevention or management of illness, functional disability, related changes in health status, and on medications and treatments.
• Evaluates the results of clinical interventions to measure progress toward goals.
• Reports all pertinent information about client status and progress to appropriate multidisciplinary health team members.
• Plans sequence of home visits with attention to issues of acuity, geographical location, and other pertinent considerations.

Requirements

• Graduate of a program in Physical Therapy approved by the American Physical Therapy Association.
• Current New York State Licensure.
• Two (2) years at senior level in field or acute medical hospital.
• Two (2) years supervised clinical experience in a medical setting preferred, or equivalent combination of education and experience.
• Must have a valid driver’s license and ability to provide own transporting during working hours for the purpose of making home visits.
