Per Diem Transitional Counselor- YAS

What You’ll Do

• Provide hands-on support with daily living skills such as personal care, meal planning, cooking, cleaning, money management, and community integration.
• Aid young adults in developing and implementing individualized recovery plans, coordinating with treatment teams, natural supports, and community resources.
• Deliver counseling, emotional support, and skill-building focused on independence, self-esteem, and quality of life.
• Support residents through crisis intervention and ensure safety and well-being at all times, including overnight shifts.
• Maintain accurate documentation, progress notes, and database entries.
• Provide transportation and accompany residents to appointments, YAS events, and activities as needed.
• Collaborate with clinical teams, nursing services, and outside providers to ensure comprehensive care.
• Stay engaged, awake, and alert during all shifts, ensuring a safe and supportive environment.

• Understanding of (or willingness to learn) Motivational Interviewing, trauma-informed practices, and integrated treatment for co-occurring disorders.
• Experience in residential, mental health, or human services preferred (training provided).
• Valid driver’s license and reliable transportation required.

Qualifications

• MINIMUM QUALIFICATIONS: Two years of college education (human services or behavioral science preferred) or High School Diploma or Equivalent and 2 years direct care work experience in behavioral health residential care.
• Must have and maintain a valid driver’s license.
• Must maintain an acceptable driving record and be insurable at a reasonable rate under the Organization’s auto insurance plan.
• Must be proficient in Microsoft 365.
• Must be able to navigate through electronic health records system (EHR).
• LICENSURE / CERTIFICATIONS: Ability to obtain and maintain Basic Life Support (BLS) certification.
