Transitional Counselor

Key Responsibilities

  • Aid residents with independent living skills, including meal planning, cleaning, hygiene, and money management.
  • Support clients in developing and following recovery plans, coordinating with treatment teams and natural supports.
  • Provide advocacy, referrals, and connections to community resources.
  • Facilitate rehab groups and interventions focused on skill building and recovery.
  • Supervise self-medication protocols and ensure accurate documentation.
  • Provide crisis intervention and collaborate with emergency services as needed.
  • Maintain clear communication through progress notes, logs, and required records.
  • Ensure a safe and supportive environment, including overnight coverage when assigned.
  • Provide transportation and assist with scheduling personal appointments.

Qualifications

  • Two years of college education or High School Diploma or Equivalent and 2 years’ direct care work experience.
  • Must be proficient in Microsoft 365.
  • Must be able to navigate through electronic health records system (EHR).
  • Must have and maintain a valid driver’s license.
  • Must maintain an acceptable driving record and be insurable at a reasonable rate under the Organization’s auto insurance plan.
  • Ability to obtain and maintain CPR/AED/First Aid training.
