Responsibilities
- Provide social services to residents, including counseling, case management, and support groups
- Develop and implement individualized care plans to meet resident needs and goals
- Collaborate with interdisciplinary care teams to ensure comprehensive care
- Conduct assessments, evaluations, and progress reports to monitor resident progress
- Identify and address resident concerns, complaints, and issues in a timely and effective manner
- Maintain accurate and detailed records of resident interactions and services provided
