About the role
The Per Diem RN plays a crucial role in providing outstanding patient care and high-level customer service in Chapters Health communities. This position is more than just a job; it's a mission to support terminally ill patients and their families.
Responsibilities
- Achieve patient/family needs through the nursing process and coordinate with the Interdisciplinary Group (IDG).
- Provide palliative and supportive care to the patient/family unit.
- Complete clinical admissions for all Company services, addressing patient/family fears and concerns.
- Initiate skilled nursing interventions, obtain physician orders, and document care according to Company policy.
- Facilitate referrals to all service lines, communicate with case managers, physicians, and the general public, and prepare for patient discharges.
- Communicate with after-hours staff, discuss potential needs, and develop strong relationships with case managers, physicians, and other professionals.
- Manage the emotional stress of working with terminally ill patients and their families, and provide bereavement interventions.
- Present concise and pertinent reports to the Interdisciplinary Group (IDG) and supervise patient care given by other nursing team members.
- Participate in patient deaths, oversee body disposal, and manage emergency situations as needed.
Qualifications
- Current RN license in the state where working.
- Active BLS for healthcare professionals from the American Heart Association or Red Cross.
- One (1) year nursing experience, preferably in hospice or hospital settings.
- Valid driver’s license and automobile insurance.
- Ability to use equipment with visual and auditory mechanisms.
- Effective verbal and written communication skills.
- Flexible schedule to meet the needs of Hospice House patients.
- Ability to manage emotional stress and physical demands related to the job.
- Completion of Alzheimer's Disease and Related Dementias Training through the Florida Department of Elder Affairs.
