Per Diem RN Home Health Visits- Orange/Osceola/Seminole County

About the Role

Registered Nurse (RN) providing home health visits in Orange, Osceola, and Seminole Counties. The RN will deliver the highest quality of care to patients utilizing the Plan of Care to ensure patient safety, well-being, and satisfaction. Experience with WellSky/Kinnser, Oasis, and Home Health is a plus.

Responsibilities

• Provides safe and effective care for the client based on individualized Plan of Care under the supervision of a Director of Nursing (DON).
• Communicates promptly about changes in patient medical status or emergencies to the team for effective case management.
• Delivers safe and effective nursing care within the scope of practice and individualized Plan of Care, in collaboration with the Nurse Supervisor/Manager of Clinical Practice. Duties may include:
  • Medication administration
  • Wound care
  • Infusion therapy
  • Patient transferring and positioning
  • Catheter care
  • Effective communication with the client, family, and Team Case Manager
• Demonstrates empathy, caring, and self-confidence; pays attention to details for thorough follow-through.
• Anticipates problems, prioritizes tasks, and demonstrates a sense of urgency.

Requirements

• Current Florida RN license.
• Possess wound care skills.
• Excellent communication, teamwork, interpersonal, collaboration, and customer service skills.
• Proficiency in using electronic health records for documentation and communication regarding client care.
• 1 year of Home Health experience preferred.

Qualifications

• Current RN license in Florida.
• Home Health experience or willingness to learn.
• OASIS experience or willingness to learn.
• Strong clinical skills.
• Excellent communication, collaboration, and documentation skills.
• Familiarity with WellSky/Kinnser/AlayaCare is a plus.
