About the role

Per Diem Field LPN providing home health visits on a part-time, per-visit basis with rates starting at $35. The role focuses on delivering high-quality care to patients according to their individualized Plan of Care, ensuring safety, well-being, and satisfaction. Experience with WellSky/Kinnser and Home Health is a plus.

Responsibilities

• Provides safe and effective care for the client based on the individualized Plan of Care.
• Communicates promptly about changes in patient medical status or emergencies to the team for effective case management.
• Delivers safe and effective nursing care within the LPN scope of practice, in collaboration with the Nurse Supervisor/Manager of Clinical Practice. Duties may include:
  • Medication administration
  • Wound care
  • Infusion therapy
  • Patient transferring and positioning
  • Catheter care
  • Effective communication with the client, family, and Team Case Manager
• Demonstrates empathy, caring, and self-confidence; pays attention to details for thorough follow-through.
• Anticipates problems, knows when to diffuse or escalate situations, and prioritizes tasks with a sense of urgency.

Requirements

• Current LPN license in the state of Florida.
• Proficiency in wound care.
• Excellent communication, interpersonal, and collaboration skills.
• Strong customer service skills.
• Ability to use electronic health records for documentation and communication regarding client care.
• 1 year of Home Health experience preferred.
