Per Diem - RN Clinical Liaison

About the role

The Admission RN plays a crucial role in Chapters Health System, ensuring a seamless transition for patients into the organization's services. This position involves assessing patients, initiating care, and serving as an ambassador for the organization.

Responsibilities

• Explains services to potential patients/families and addresses their questions
• Identifies admission objections and provides responses
• Edits informed consents and initial assessments
• Communicates with physicians to initiate Plan of Care
• Initiates skilled nursing interventions
• Obtains physician orders
• Completes documentation according to Company policy
• Facilitates referrals to all service lines
• Communicates with Care Navigation staff for timely follow-up
• Provides clinical communication using SBAR
• Discusses potential needs with after-hours staff
• Obtains consents for services and arranges safe discharge
• Develops relationships with case managers, physicians, etc.
• Provides care for GIP patients in a hospital
• Performs other duties as assigned

Qualifications

• Current license as RN in the state where working
• Active BLS for healthcare professionals from the American Heart Association or Red Cross
• Minimum of one (1) year of RN experience; hospice or hospital experience preferred
• Strong clinical assessment skills with preferred background in emergency department or ICU
• Previous experience working with an EMR/EHR system
• Effective verbal and written communication skills; bi-lingual in Spanish preferred
• Valid driver’s license and automobile insurance per Company policy
• Reliable transportation to meet schedule
• Ability to work in a team environment and handle emotional stress
• Ability to manage multiple priorities simultaneously
• Ability to perform essential functions and physical requirements (lifting, bending, pushing/pulling, kneeling) with or without reasonable accommodation
• Strong goal/outcome orientation with exceptional time management skills
• Excellent customer service focus
