Clinical Liaison (RN or RT) - Per-Diem

Key Responsibilities

• Work with physicians, discharge planners, and patients' families to plan the best continuum of care after an ICU or post-surgical stay
• Utilize your strong clinical background, payer knowledge, and problem-solving skills to perform clinical assessments of referred patients
• Counsel with patients' families to assist them in making the best care decisions for their loved one
• Partner with hospital case managers and physicians to create a smooth pre-admission and transfer process by removing payer-related and operational barriers for patients getting into our highly effective level of care
