Per Diem RN and LPN Opportunities

About the role

The Neville Center at Fresh Pond is a Joint Commission and American Healthcare Association award-winning short-term rehab, long-term, and skilled nursing care community located on the Cambridge/Belmont line. We are dedicated to our residents and team members and are currently seeking highly motivated and qualified per diem (PRN) RN and LPN's to join our nursing department.

Responsibilities

• Assessing the health care needs of the resident, making a nursing diagnosis, transitioning this through collaboration with the nursing team into an interdisciplinary plan of care utilizing all resources available.
• Communicate this plan to all care providers in a way that ensures continuity of care for residents.
• Establish and maintain functional communications with staff, other disciplines, residents, families, medical staff and community.
• Able to assess the effect of medications and treatments and monitor these effects to provide appropriate feedback to interdisciplinary team and medical managers to continuously provide the resident with the treatment appropriate to their individual needs.
• Use of the Electronic Health Record - PCC preferred.
• Lead and direct the nursing staff in the safe, effective delivery of care by providing a structure.
• Provide education to residents regarding their healthcare issues and care plan strategies.
• Knowledge and ability regarding Infection control, fire and disaster policies as well as other facility and/or state and federal policies and practices that provides a safe environment for residents, community and staff.
• Knowledge of and ability regarding the protection of resident rights, and the treatment of residents, staff and community with respect and dignity.
• Treating resident and facility property and information with honesty, confidentiality and respect.

Requirements

• A weekend, at least once a month shift required.

Qualifications

• Registered Nurse (RN) and Licensed Practical Nurse (LPN).

Skills

• Knowledge of Electronic Health Record (EHR).

Pay

• RN: $51/hour, up to $57 with shift differential
• LPN: $46/hour, up to $50 with shift differential

Schedule

• A weekend, at least once a month shift required.
