About the role

Vinfen is a nonprofit organization providing community-based services to individuals with various disabilities and challenges. This position involves working with adults who have brain injuries and managing their complex health needs.

Responsibilities

  • Participates in comprehensive assessments of physical health upon admission and with any changes in medical status.
  • Provides nursing care, teaches, and assists in daily living activities according to the care plan.
  • Maintains and monitors clients for any changes in status, implements Individualized Action Plans (IAPs), and uses recovery-oriented rehabilitation approaches.
  • Communicates any changes in status to relevant parties, performs assessments with changes, and provides skilled nursing interventions as needed.
  • Takes and documents vital signs, collects samples, administers medications, and performs treatments as directed.
  • Maintains medical equipment and supplies, provides transportation to appointments, participates in trainings, and adheres to company policies and procedures.
  • Contributes to quality improvement goals, uses crisis intervention techniques, and ensures site safety.
  • Completes incident documentation and performs other related duties as required.

Qualifications

  • High school diploma or GED.
  • Current L.P.N. license in Massachusetts.
  • Experience working with people with brain injury preferred.
  • Strong time management and writing skills.
  • At least 21 years old and holds a valid US driver's license for at least 6 months.
  • Must be able to obtain CPR certification within 2 weeks and Safety Care certification within 90 days of employment.
  • Passes CORI, reference, and national fingerprinting background checks.
