Per Diem Licensed Practical Nurse

About the role

AccordCare is seeking a Per Diem Licensed Practical Nurse (LPN) who is committed to delivering compassionate, skilled care in the home setting. This role offers the opportunity to provide one-on-one support that promotes client health, independence, and dignity.

Responsibilities

• Provide skilled nursing care tailored to each client’s unique needs
• Maintain health conditions and document changes in clinical status
• Administer medications and perform wound care
• Deliver G-tube, trach, or vent support when required
• Educate and guide clients and families to support ongoing health at home
• Communicate effectively with the care team to ensure continuity of care

Requirements

• Licensed as an LPN and in good standing with the Georgia Board of Nursing
• Experienced in a clinical setting, with at least one year preferred
• Certified in CPR
• Skilled in EMR documentation, with strong organizational ability
• Confident working independently in a home environment
• Skilled in client-centered care and communication
• Experienced with G-tube, trach, or vent care

Qualifications

• Experience in a home health setting preferred
• Strong interpersonal and communication skills
• Ability to work flexible hours, including evenings and weekends
