About the role

Ennoble Care is a mobile primary care, palliative care, and hospice service provider serving multiple states including Oklahoma. We provide comprehensive care to patients with chronic conditions and limited mobility, focusing on remote patient monitoring, behavioral health management, and chronic care management.

Responsibilities

  • Conduct a psychosocial assessment within 5 days of admission to the hospice program.
  • Participate in IDG meetings and develop the patient’s plan of care.
  • Observe, record, and report changes in the patient's emotional and social factors.
  • Maintain and submit written clinical records as required by the agency.
  • Evaluate the patient's and family's response to social work intervention.
  • Schedule visits with the patient's Case Managers to facilitate coordination of other staff visits.
  • Participate in IDG and QAPI meetings and activities.
  • If the patient/family declines Social Work services, continue to offer support and monitor evolving needs.
  • Assist in discharge planning, as needed.
  • Strive to improve practice through continuous learning and engagement in professional activities.
  • Assess the patient's and family members' coping abilities with the patient's illness.
  • Consult with the health team to understand social, emotional, and environmental factors affecting the patient's health.
  • Help patients access family and community resources.

Qualifications

  • A graduate of a master’s program in social work (MSW) accredited by the Council on Social Work Education.
  • A graduate of a bachelor’s program in social work accredited by the Council on Social Work Education, meeting personnel qualifications stated in §418.114(b)(3).
  • Minimum of one (1) year’s experience in health care, with hospice experience preferred.
  • Understanding of hospice philosophy and issues of death/dying.
  • Experience in a hospice care setting preferred.
  • Good verbal and written communication, and organizational skills.
  • Licensed driver with an insured vehicle in good condition.

Pay and Benefits

The pay rate is $40.00 per visit and $65.00 per admission. Full-time employees qualify for medical, dental, vision, and supplementary benefits such as life insurance, short term and long term disability, flexible spending accounts, accident, critical illness, and hospital indemnity.
