Per Diem Hospice Registered Nurse

About the Role

AdvisaCare is seeking a compassionate, dynamic, and organized Per Diem Hospice RN for our Flint location servicing the surrounding areas. This challenging and rewarding position requires a registered nurse who sees the value of hospice care, providing emotional support to help patients live their final days with dignity and comfort.

Responsibilities

• Conduct appropriate supervisory visits allowing for clinical observation of performance of assigned team members
• Participate and provide feedback to the team for regular interdisciplinary group meetings
• Contribute to the development, review, and revision of the patient/family plan of care
• Perform initial and ongoing assessment of patient's physical, functional, psychosocial, and environmental needs
• Implement the individualized plan of care and revisions as needed
• Ensure compliance with AdvisaCare Hospice Policies and Procedures and Medicare compliance
• Consult with and educate the patient/family on disease process, self-care techniques, end-of-life care, and dealing with issues of ethical concern
• Supervise paraprofessionals providing services to patients
• Prepare clinical and progress notes
• Participate in IDG conferences
• Participate in Hospice performance improvement program
• Maintain high level of customer satisfaction with clients, families, and caregivers
• Communicate information using current process and technology within the organization

Requirements

• Current Michigan License in good standing
• Minimum 1 year hospice experience
• Empathetic approach to patients and caregivers
• Ability to participate in field visits daily and weekly office meetings as indicated
• Ability to adapt to patient's status and needs on a full-time on-call basis
