Hospice RN Case Manager

Journey Hospice is seeking a per diem RN Case Manager to oversee primary caseloads of Hospice patients and provide skilled nursing care. This role offers flexibility in scheduling with a potential pay rate up to $47/hour.

Responsibilities

• Oversee primary caseload of Hospice patients
• Provide skilled professional nursing care to patients, their families, and caregivers as prescribed by the physician
• Develop and implement the Hospice Interdisciplinary Team (IDT) plan of care
• Teach, supervise, and counsel patients, family members, and staff regarding nursing care and patient needs
• Work closely with other members of the IDT to ensure all patient and family care needs are being met

Requirements

• Graduate of an accredited registered nursing program
• Bachelor’s degree in Nursing preferred
• Certified Hospice and Palliative Nurse (CHPN) certification preferred
• NJ RN Licensure by the NJ State Board of Nursing in good standing
• Valid drivers license and reliable transportation that is insured in accordance with NJ state requirements

Qualifications

• One (1) year general nursing preferred with current hospice, home health, medical, surgical, or critical care experience
• Experience with pain and symptom management interventions preferred
• Ability to work within an interdisciplinary setting
• Understanding of Hospice philosophy and principles of death/dying
